Connecticut Institute for Communities seeks an Internal Medicine Nurse to provide comprehensive adult care in our community-focused health centers. In this role, you will assess patients, manage chronic conditions, administer medications, and coordinate care for diverse, often underserved populations. You’ll collaborate closely with physicians and care teams, provide patient and family education, and support community health initiatives. Deliver comprehensive adult internal medicine nursing care to diverse, underserved patients.
Perform assessments, develop and update care plans, and monitor chronic conditions.
Administer medications, treatments, and coordinate diagnostic testing.
Educate patients and families on disease management, prevention, and medications.
Collaborate with physicians, social workers, and allied health staff on care coordination.
Document care thoroughly in EHR and support quality and safety initiatives.
Participate in community health programs and population health outreach.
Contribute to a culture of inclusivity, continuous improvement, and public service.
